29 CFR 1926.20(b) Accident Prevention Responsibilities: (2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about March 13, 2026, the employer failed to ensure that frequent and regular inspections of the job site were conducted. A floor opening without a cover was not corrected. As a result, three employees performing work around the floor opening were exposed to fall hazards of approximately 11'.

As a result, employees were exposed to fall hazards of approximately 11 feet into the floor opening to the concrete floor below.

29 CFR 1926.1053(b) Use: (15): Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. a) On or about March 13, 2026, the employer failed to ensure that a podium ladder was inspected by a competent person on a periodic basis in that an employee was using a damaged (broken latch) ladder as the means of access and egress to an interior wall to grind a piece of steel overhang. As a result, an employee fell from the ladder, through a floor opening, approximately 17' to the concrete floor below.
