29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  Site: 11 Bay 20th Street Brooklyn, NY  On or about 5/31/19  a) Employer did not ensure a competent person inspected jobsite, materials and equipment prior to employee working. Employees used a Werner extension ladder that was defective; bent rungs and did not extend 3 feet past the intended landing. The employees were also exposed to fall hazards, they were not trained in fall protection.

29 CFR 1926.502(e)(2): Positioning devices were not secured to an anchorage capable of supporting at least twice the potential impact load of an employee's fall or 3,000 pounds (13.3 kN), whichever was greater.  Site: 11 Bay 20th Street Brooklyn, NY  On or about 5/31/19  a) An employee removing form work fell when the flat eye anchorage broke.  The employee fell approximately 15 feet.

Site: 11 Bay 2oth Street Brooklyn, NY  On or about 5/31/19  a) An employee used a 15 feet Werner ladder to access a form wall. The ladder did not extend 3 feet past the intended landing.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately.  Site: 11 Bay 20th Street Brooklyn, NY  On or about 5/31/19  a) Employee used an extension ladder with structural defects to access work area approximately 15 feet above foundation. Several rungs on the ladder were damaged.
