29 CFR 1926.651(h)(1):  Employees were working in an excavation in which there was accumulated water or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:    On or about 5/23/2019, employees of Lehman Construction LLC were installing a 24 inch storm pipe near the intersection of West Pearce Boulevard and Goodfellow Road, Wentzville, MO.  Water was allowed to accumulate in the trench to a depth of approximately three inches.  The accumulation of water jeopardized the structural integrity of the trench, exposing employees to engulfment and cave-in hazards.
