29 CFR 1926.703(a)(1): Formwork was not designed, fabricated, erected, supported, braced and maintained so that it would be capable of supporting without failure all vertical and lateral loads that may reasonably be anticipated to be applied to the form:  On or about October 26, 2022, Employees engaged in the construction of a continuous concrete bridge at the job site located at NE 148th St Bridge Carroll Creek, Kearney, Missouri were exposed to fall and struck-by hazards while employees were pouring concrete on the bridge deck, in that the design and inspection of the falsework by employer and/or employer?s subcontractors or others working on the project did not conform with the falsework plans.
