29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system : a- Roof Exterior: Employees engaged in roofing activities on a 8 on 12 pitched roof were exposed to fall hazards of approximately 12 & 20ft falls to the concrete below as a means of fall protection was not utilized. Condition noted on or about 05/29/2019. Division Construction Inc was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13): which was contained in OSHA Inspection Number 1139213, Citation # 1, Item # 1, issued on 05/18/2016 with respect to a work place located at 363 Johnson Ave, Englewood NJ 07631. This citation became a final order on (6/16/2016).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a- Roof Exterior: Employees engaged in roofing activities on a 8 on 12 pitched roof were exposed to fall hazards of approximately 20ft falls to the concrete below as an extension ladder used for access did not extend 3ft above the upper landing surface. Condition noted on or about 05/29/2019. Division Construction Inc was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.1053(b)(1): which was contained in OSHA Inspection Number 1139213, Citation # 1, Item # 2, issued on 05/18/2016 with respect to a work place located at 363 Johnson Ave, Englewood NJ 07631. This citation became a final order on (6/16/2016).
