29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At the jobsite, on the porch roof on the front North side of the building, on or about 6/3/19: Employees were working on a 4:12 pitch roof with a ground to eave height of 11 feet without fall protection of any kind. b) At the jobsite, on the roof over a dormer on the third story at the rear of the building, on or about 6/3/19: Employees were working on a 4:12 pitch roof with a ground to eave height of 30 feet without fall protection of any kind. c) At the jobsite, on the roof over a dormer on the third story at the rear of the building, on or about 6/3/19: Employees returned to work on this roof surface, following the opening conference and wearing harnesses, worked without fall protection (attaching to an anchorage).

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee a a) At the jobsite, on the roof over a dormer on the third story at the rear of the building, on or about, on or about 6/3/19: An anchorage was installed where two snap-hooks and lifelines were attached to provide fall protection to the workers on the ladder jack scaffolds below this surface. This equipment is not designed for two attachments and will not ensure 5,000 lbs of strength.
