The employees were working on all areas of the roof and the employer did not ensure that they were trained how to recognize and minimize fall hazards.

On or about June 4, 2019, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 1209 Berwin, Akron, Ohio 44310. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.
