29 CFR 1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level; On and about June 4, 2019, employees were exposed to falls while reroofing a commercial building when they were working outside of a 15 degree angle from their anchor points which would allow them to fall >6' and potentially hit the ground.

29 CFR 1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head. On and about June 4, 2019, employees were exposed to serious injury while reroofing a commercial building when they were connecting their lifelines to their harnesses using the waist D-rings.
