29 CFR 1910.244(a)(2)(i): Where there was a possibility of slippage of the cap, a block was not placed in between the cap and the load: On June 4, 2019 an employee was exposed to crush-by hazards while jacking up the front end of a semi tractor and the employer did not ensure that a block had been placed between the load and the cap to prevent slipping.

OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees On or about June 4, 2019, though the employer had provided its mechanics with informal training on the importance of working alongside, rather than under, loads whenever feasible, the employer had not included the specific instructions of working alongside loads when feasible as part of its formal training program.
