The employee was working at an approximate height of 10 foot while only standing on the narrow bolt flange that was not large enough to accommodate footing for his entire foot.  No fall protection was provided or utilized.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
