29 CFR 1910.110(f)(6)(ii): Containers were not stored in a suitable enclosure or otherwise protected against tampering. Water Treatment Center, adjacent to flammable storage locker; on or about 03 June 2019 and times prior thereto; the employer did not ensure that propane fuel cylinders were stored in a manner to prevent damage. A propane cylinder was stored free standing outside of flammable locker. This condition exposed employees to contact with flammable and combustible liquids and other fire hazards.

Administrative office; on or about 03 June 2019 and times prior; the employer did not provide the OSHA Form 300A Summary of Work-related Injuries and Illness and OSHA Form 300 Log of Work-Related Injury and Illness Logs for 2016, 2017, and 2018 within four business hours. Instance (a) For year 2016, OSHA Form 300 and 300A or its equivalent was not provided. Instance (b) For year 2017, OSHA Form 300 and 300A or its equivalent was not provided. Instance (c) For year 2018, OSHA Form 300 and 300A or its equivalent was not provided. This condition leads to delays in collecting of injury and illnesses data.
