A flight of stairs less than 44-inches, with nine risers did not have a handrail installed. This condition exposed employees to a fall hazard.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. Machine shop, on or about 04 June 2019 and times prior thereto; the employer did not ensure that the work rest was closely adjusted to less than one-eighth of an inch from the abrasive wheel. A Milwaukee Bench Grinder, model number 4991, serial number W6-98 on the left and right hand sides, the work rest was approximately 1/2-inch from the abrasive wheel. This condition exposed employees to a caught in hazard.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage. Machine shop, East wall; on or about 04 June 2019 and times prior thereto; the employer did not ensure that the electrical panel was readily accessible in the event of an electrical emergency. A work bench and other miscellaneous items were stored in front of the panel impeding immediate access. This condition exposed employees to an electrical shock and electrical burn hazards.
