Maintenance facilities; on or about 05 June 2019 and times prior thereto; two instances were noted where bench mounted grinders did not have the work rest closely adjusted to within one-eighth of an inch to the abrasive wheel: a. Maintenance Garage; a Delta ten-inch bench mounted grinder, model 23-980, serial number 9903; did not have the work rest adjusted to less than one-eighth of an inch from the abrasive wheel. On the left-hand side, the distance from the work rest to the abrasive wheel was approximately one-quarter of an inch. b. Trails Shop; a Wissota bench mounted grinder, model E-7, serial number W8-82 did not have the work rest adjusted to less than one-eighth of an inch from the abrasive wheel. On the left-hand side, the distance from the work rest to the abrasive wheel was approximately three-quarters of an inch. This condition exposed employees to a caught in hazard.

Trails shop workbench; on or about 05 June 2019 and times prior thereto; the employer did not ensure that a receptacle outlet was installed wherever flexible cords with attachment plugs are used. An energized orange 50-foot medium duty extension cord was routed to a four-place electrical outlet box to supply power to the workbench area. The four-place electrical outlet box had non-metallic electrical cable, with a plug attached to receive power from the extension cord; which was used to energize the four-place electrical outlet box. The four-place electrical outlet box had a Wissota model E-7, serial number W8-82, bench mounted grinder plugged into the four-place electrical outlet box to receive power. This condition exposed employees to an electrical shock and electrical burn hazard.
