29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  A) The Outershell LLC on or about June 7, 2019, at the address of 7611 Red Pines Dr. Sylvania, Ohio: The employer did not have a plan or program to identify fall hazards which assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls, commonly associated within their scope of work, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  A) The Outershell LLC on or about June 7, 2019 at the address of 7611 Red Pines Dr. Sylvania, Ohio: The employer did not ensure employee's had been trained in the appropriate use of personal fall arrest systems or alternative measures while performing work tasks from elevated surfaces. Employees were exposed to falls of approximately 18 feet to the ground below during re-roofing operations.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b).  A) The Outershell LLC on or about June 7, 2019 at the address of 7611 Red Pines Dr. Sylvania, Ohio: The employer did not ensure employee's were using the appropriate use of personal fall arrest systems or alternative measures while performing work tasks from elevated surfaces. Employees were exposed to falls of approximately 18 feet to the ground below during re-roofing operations.
