Two employees were working on a 8/12 pitched residential roof performing shingling. One worker was working in close proximity to the eave of the roof without any means of fall protection. The other worker was traveling back and forth on the roof laying out shingles and other material.  The Outer Shell LLC was previously cited for a violation of an equivalent Occupational Safety and Health standard 1926.20(b)(1) which was contained in OSHA inspection number 1406455, citation number 1, item number 1-a and was affirmed as a final order on July 23, 2019, with respect to a workplace located at 7611 Red Pines Drive, Sylvania, Ohio.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)	The Outer Shell LLC worksite located near 1005 Stoneleigh Rd, Perrysburg, OH, 43551:  On or about March 4, 2021, the employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below. Employees were exposed to falls of approximately 18 feet to the ground below during roofing operations. Two workers were on the roof, one worker was shingling right next to the eave of the roof without means of fall protection.  The Outer Shell LLC was previously cited for a violation of an equivalent Occupational Safety and Health standard 1926.503(a)(1) which was contained in OSHA inspection number 1406455, citation number 1, item number 1-b and was affirmed as a final order on July 23, 2019, with respect to a workplace located at 7611 Red Pines Drive, Sylvania, Ohio.
