29 CFR 1910.28(b)(8)(ii): The employer did not apply floor markings at least 6 feet (1.8 m) from the edge of the pit in colors that contrast with the surrounding area; or places a warning line at least 6 feet (1.8 m) from the edge of the pit as well as stanchions that are capable of resisting, without tipping over, a force of at least 16 pounds (71 N) applied horizontally against the stanchion at a height of 30 inches (76 cm); or places a combination of floor markings and warning lines at least 6 feet (1.8 m) from the edge of the pit;  On or about June 07, 2019 the employer provided a vehicle service pit which was 36 inches wide in it's least dimension and measured 4 feet, 3/4 inch deep the employer placed combination of floor markings and warning lines 4 feet from the pit with warning line stanchions which were not capable of resisting, without tipping over, a force of at least 16 pounds applied horizontally at a height of 30 inches.

On or about June 07, 2019 the employer provided a vehicle service pit which was 36 inches wide in it's least dimension and measured 4 feet, 3/4 inch deep with stairs entering the pit having six risers and five, 36 inch wide treads, the employer did not provide a handrail on at least one side of the stairs entering the northeast end of the vehicle service pit.  b.  On or about June 07, 2019 the employer provided a vehicle service pit which was 36 inches wide in it's least dimension and measured 4 feet, 3/4 inch deep with stairs entering the pit having six risers and five, 36 inch wide treads, the employer did not provide a handrail on at least one side of the stairs entering the southwest end of the vehicle service pit.

The employee was treated at the Gwinnett Medical Center receiving a prescription for pain medication which he reported to his supervisor two days later and the employer did not record the injury on the OSHA form 300 or equivalent form.    b.
