29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)   A written hazard communication program was developed for employees that were using hazardous chemical products such as but not limited to; acrylic latex paints (Sherwin Williams, Henry), flammable Startex paint thinner, Polyblend grout, and concrete containing Portland cement.  Employees were exposed to chemical hazards.

Safety Data Sheets (SDS) were not maintained on site for various types of acrylic latex paints (Sherwin Williams, Henry), flammable Startex paint thinner, Polyblend grout, and concrete containing Portland cement that the employees were using.  Employees were exposed to chemical hazards.

29 CFR 1926.403(b)(2): Installation and use. Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification.   An electrical receptacle box (110V) designed to be used mounted in a wall was used to energize electrical hand tools such as saws and drills was dangling above ground from a rope.  Employees were exposed to electrical hazards.

Outdoor Shop, GCC:  The faceplate on the temporary electrical receptacle (110V) used to energize hand held tools such as saws and drills did not completely cover the receptacle box.  Employees were exposed to electrical hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      Pump Room:  The portable straight ladder used to access the scaffolding did not extend at least 3 feet above the landing surface, nor was it secured at the top. Employees were exposed to fall hazards in excess of 10 feet.
