29 CFR 1910.147(c)(4)(ii)(B): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    a) Facility: On or about 4/9/19, and at times prior, the employer exposed employees to amputation hazards in that there were no specific lockout procedures to control hazardous electrical and hydraulic energy during servicing equipment such as a 320 ton Accurpress Column Punch Model 73208 S/N 7559.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available within 10 working days of initial assignment to all employee(s) with occupational exposure: Note: Does not apply to employees who have previously received the complete hepatitis B vaccination series, for whom antibody testing has indicated immunity, or for whom the vaccine is contraindicated for medical reasons.    (a) Office: On or about 4/9/19, and at times prior, the employer exposed employees to biological hazards, in that Hepatitis B vaccinations or antibody testing for immunity was not provided to employees performing First Responder duties, during their initial 10 days of occupational exposure.
