Worksite: The employer did not ensure that all fall protection systems used by employees were inspected for wear, deterioration or damage prior to use. On or about 06/04/2019, an employee fell onto the deck due to worn and aged fall arrest device worn by the employee while working on a residential roof, and the employee sustained serious injuries from the incident.
