Boiler Room: Employees use a fixed ladder to access the HVAC equipment in order to change air filters. The ladder has only one side rail that extends beyond the access level. Employees are exposed to approximate 14-foot fall hazards.

The U.S. Department of Interior, National Park Service, Olympic National Park, U.S. Geological Survey Building was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.157(e)(2)], which was contained in OSHA inspection number 1313402, citation number 2, item number 2A and was affirmed as a final order on June 12, 2018, with respect to a workplace located at 600 East Park Avenue, Port Angeles, Washington 98362.

The work rest measured 1/4 inches from the abrasive wheel at the time of the inspection. Employees were exposed to entanglement hazards.   The U.S. Department of Interior, National Park Service, Lake Mead National Recreation Area - Natural Resources, was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.215(a)(4)], which was contained in OSHA inspection number 1221428, citation number 1, item number 3 and was affirmed as a final order on September 27, 2017, with respect to a workplace located at 101 Katzenbach Drive, Boulder City, Nevada 89005.  The U.S. Department of Interior, National Park Service, was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.215(a)(4)], which was contained in OSHA inspection number 1165276, citation number 1, item number 2 and was affirmed as a final order on October 6, 2016, with respect to a workplace located at 64100 Generals Highway, Sequoia National Park, California 93262.
