29 CFR  1910.151(c):  The employer did not ensure where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.4 The eyewash unit shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment.  Administrative Building:  On or before April 26, 2022, employees were handling corrosive chemical materials such as Clorox Bleach, 3M 8H concentrate, and 3M 24H concentrate cleaners where an emergency eye wash station in close proximity for quick drenching or flushing of the eyes was not available within the area for immediate emergency use. Employees were exposed to severe eye burns or blindness.

29 CFR  1910.134(d)(1)(iii):  The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  Upper Maintenance:  On or before April 26, 2022,  an employee conducted a non-routine welding project on stainless steel metal bars forming a metal bench used inside the park.  The employer did not evaluate the respiratory hazard of metal fumes. This condition exposed employees to upper respiratory illnesses and lung damage.

29 CFR  1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:  Maintenance:  On or before April 26, 2022,  an employee conducted non-routine restroom remodeling projects and routine carpentry tasks and wore a 3M N95 face filtering respirator without being fit tested prior to initial use.  The employer did not provide an initial fit test for respirators used. The employee was potentially exposed to upper respiratory irritation from Portland Cement and wood dust.

The following locations:  a)  Custodial Closet:  Employees used  safety glasses instead of tight fitting safety goggles while using concentrated Clorox Bleach, 3M 8H concentrate, and 3M 24H concentrate cleaners during cleaning operations.   b)  Groundskeeping:  Employees used  safety glasses instead of tight fitting safety goggles while spraying herbicides from backpack equipment.  c)  Lower Maintenance:  Employees conducted multiple non-routine tasks such as concrete mixing with dry products, concrete forming, leveling, and shower renovation projects using chemicals without wearing elbow length chemical resistant gloves.  Employees were exposed to severe eye burns, blindness, lacerations, abrasions, and dermal burns.

The employer did not provide personal protective equipment (PPE) training specific to the hazardous chemical.  The employees were exposed to dermatitis, abrasion, dermal burns, and upper respiratory inhalation hazards.
