On or about June 11, 2019, on the 3rd floor of an open balcony, an employee performing flooring installation was not protected from a 23 feet 7-inch fall hazard by use of a guardrail system, safety net system or personal fall arrest system. b. On or about June 11, 2019, on the 4th floor of an open balcony, an employee performing flooring installation was not protected from an 32-foot 7-inch fall hazard by use of a guardrail system, safety net system or personal fall arrest system.
