The employer had a make-shift power strip made out of a extension cord into a metal junction box with 2 regular household outlets that were not grounded.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  (a)    Worksite located at 712 W 45th St, KC MO -- Employees were exposed to electrical burns/shock hazards while using an extension cord that was missing a ground pin and had been spliced for repair.

JCN Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1131362, citation number 1, item number 1 and was affirmed as a final order on 03/14/2016, with respect to a workplace located at 10430 NE 106th St, KC MO.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)     Worksite located at 8031 NW Milrey Dr, KC MO -- Employees were exposed to fall hazard in that the ladder being used to access the roof level was not extended at least 3 feet above the upper landing surface.  Hixon Brothers Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1131362, citation number 1, item number 3, and was affirmed as a final order on 03/14/2016, with respect to a workplace located at 10430 NE 106th St, KC MO.
