Power station, on or about 11 June 2019 and times prior thereto; the employer did not ensure that stairways with four or more risers' was equipped with a handrail on each enclosed side. A set of stairs, 54-inches wide, with six risers did not have a handrail installed on the enclosed side. This condition exposed employees to a fall hazard.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. Carpentry shop; on or about 11 June 2019 and times prior thereto; the employer did not ensure that the work rest on bench mounted grinders was adjusted to less than one-eighth of an inch from the abrasive wheel. A Craftsman bench mounted grinder, model 397-19580, serial number C-2371 was approximately 1-inch from the work rest to the abrasive wheel. This condition exposed employees to a caught in hazard. The National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(a)(4) which was contained in OSHA inspection number 1073888, citation number 2, item number 1 and was affirmed as a final order on 08 August 2015, with respect to a workplace located at Denali National Park.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. C-Camp; on or about 11 June 2019 and times prior thereto; the employer did not ensure that the tongue guard on a bench mounted grinder was adjusted to less than one-quarter of an inch from the abrasive wheel. Two instances were noted: a. Carpentry shop, a Craftsman bench mounted grinder, model 397-19580, serial number C-2371 had the tongue guard adjusted to approximately 1/2-inch from the abrasive wheel. b. Trails Shop DeWalt bench mounted grinder, model DW758, serial number 2011-12-YL48427 had the tongue guard adjusted to approximately 1/2-inch from the abrasive wheel. This condition exposed employees to a caught in hazard. The National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9) which was contained in OSHA inspection number 1073888, citation number 1, item number 1 and was affirmed as a final order on 08 August 2015, with respect to a workplace located at Denali National Park.
