Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5: Electrical Shop: On or about June 11, 2019 and at times prior thereto, rubber gloves used for the disconnection of an energized circuit rated at 7000 volts had not been inspected since February 2017. This condition exposed the employee to electrical hazards from glove failure.
