29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Meat cutting area; on or about 10 June 2019 and times prior thereto; the employer did not ensure that the electrical cable on portable equipment, was free of damage. The electrical cord on a Bulker model 120, serial number 4890 meat grinder attachment had damage to the outer protective jacket. This condition exposed employees to an electrical hazard.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. Hallway to warehouse; on or about 10 June 2019 and times prior thereto; an energized rocker switch was damaged. Rocker switch had broken toggle. This condition exposed employees to damaged electrical equipment.
