29 CFR 1910.219(h)(1): Projecting key(s), setscrew(s), or other projections in revolving part(s) were not removed, made flush, or guarded by metal cover(s): Warehouse: On or about June 10, 2019 and at times prior thereto, eight (8) projecting rotating shaft ends with set screws on the Automated Conveyor Systems, Inc. "belt" were not guarded. This condition exposed employees to caught-in hazards.
