29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    a. On or about April 29, 2019, the employer failed to ensure that operators who were tasked with unjamming equipment and removing stuck parts were trained as authorized employees to perform such tasks.     b. On or about September 4, 2019 a maintenance employee was working on Press #2 due to an electrical issue and had not applied appropriate locks to all energy sources, including the hydraulics.  The employee was identified as an authorized employee but had not received appropriate training by the company.

29 CFR 1910.147(d):The application of energy control (lockout or tagout) actions were not done in the following sequence:    (1) prepare for shutdown,  (2) shut down machine or equipment,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tagout devices to each energy isolating device by authorized employee,  (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy,  (6) verify that isolation and deenergization of the machine or equipment have been accomplished:    a.) On or about April 29, 2019, the employer failed to ensure that Press #13 (Sterling, 16D13, serial #66076) was locked and tagged out prior to an employee entering through the gated double door in order to access the inside of the mold to retrieve a part that was stuck.  Steps 1-6 were not applied prior to the employee performing this task.      b.) On or about September 4, 2019, the employer failed to ensure the Press #2 was locked and tagged out properly prior to an employees performing electrical work on the equipment. Steps 1-6 were not applied prior to the employee performing this task.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about April 29, 2019, the employer failed to ensure that effective guarding was in place on Press Machines, including but not limited to, the Sterling Press Mold #13.  Press #13 did not have a safety mat in place, had an electrically faulty front door safety strip and no other forms of adequate guarding in place including the back side of the press to protect employees from struck-by hazards created by moving machinery during normal operations.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a.) On or about April 29, 2019, the employer failed to ensure employees were utilizing lockout/tagout procedures when entering the gated double doors in order to access the inside of the mold to retrieve jammed parts in Press #13 (Sterling, 16D13, serial #66076).    b.) On or about September 4, 2019, the employer failed to ensure employees were utilizing lock/tagout procedures when performing maintenance on Press #2. Employees failed to lock out the hydraulics on the press. Employees had only applied a lock to the main disconnect.     NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1084333, citation 1, item 2a, which was affirmed as final order on September 25, 2015, with respect to the workplace located at 1330 Atlanta Highway, Madison GA 30650.    NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1142297 citation 1, item 2, which was affirmed as final order on August 19, 2016, with respect to the workplace located at 1330 Atlanta Highway, Madison GA 30650.
