Femco Holdings LLC was hired to perform contract work at a gas plant.  The gas plant contained highly hazardous chemicals in excess of the threshold quantity for flammable gases or liquids.  The gas plant was a PSM facility.  Femco Holdings LLC did not assure that each of its employees was instructed on the hazards related to their jobs and the process and the provisions of the facility emergency action plan.  This condition exposed five employees to fire, explosion, and chemical hazards.
