29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 06/14/19, at a jobsite on a 2-story house at 249 Crestwood Avenue, Buffalo, NY: A Ladder Jack Scaffold was erected on the north side of the house. An employee was standing on the aluminum pik of that scaffold while replacing the fascia. The employee was exposed to a fall of approx. 15 ft. because no fall protection system was being utilized. NO ABATEMENT CERTIFICATION REQUIRED

Pitch of the roof was approx. 6 in 12. NO ABATEMENT CERTIFICATION REQUIRED

An extension ladder was used by an employee to access the scaffold. The side rails of that ladder only extended approx. 28 in. above the aluminum pik on the scaffold. NO ABATEMENT CERTIFICATION REQUIRED
