29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 06/14/19, at a jobsite on a 2-story house at 249 Crestwood Avenue, Buffalo, NY: A Ladder Jack Scaffold was erected on the north side of the house. An employee was standing on the aluminum pik of that scaffold while replacing the fascia. The employee was exposed to a fall of approx. 15 ft. because no fall protection system was being utilized.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 06/14/19, at a jobsite on a 2-story house at 249 Crestwood Avenue, Buffalo, NY: Two employees engaged in roofing work on the steep roof on the north side of the house were exposed to a fall of approx. 19 ft. because fall protection was not being utilized. Pitch of the roof was approx. 6 in 12.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 06/14/19, at a jobsite on a 2-story house at 249 Crestwood Avenue, Buffalo, NY: A Ladder Jack Scaffold was erected on the north side of the house. An extension ladder was used by an employee to access the scaffold. The side rails of that ladder only extended approx. 28 in. above the aluminum pik on the scaffold.   NO ABATEMENT CERTIFICATION REQUIRED
