29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 08/02/19, at a jobsite on a 2-story house at 90 Peter Street, Buffalo, NY: Two Ladder Jack Scaffolds were erected on the north side of the house. Two employees were working from the aluminum pik of the scaffold that was nearest to the back of the house. The employees were exposed to a fall of approx. 18 ft. because no fall protection system was being utilized.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another.  a) On or about 08/02/19, at a jobsite on a 2-story house at 90 Peter Street, Buffalo, NY: Two Ladder Jack Scaffolds were erected on the north side of the house. There was an approx. 6 ft. gap between the aluminum pik on the front ladder jack scaffold and the aluminum pik on the back ladder jack scaffold. That gap was bridged by putting two wood planks side by side, spanning the gap.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 08/02/19, at a jobsite on a 2-story house at 90 Peter Street, Buffalo, NY: Two employees engaged in roofing tear-off work on the steep roof on the north side of the house were exposed to a fall of approx. 20 ft. because fall protection was not being utilized. Pitch of the roof was approx. 12 in 12.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 08/02/19, at a jobsite on a 2-story house at 90 Peter Street, Buffalo, NY: A Ladder Jack Scaffold was erected on the north side of the house. An extension ladder was used by employees to access the scaffold. The side rails of that ladder extended approx. 32 in. above the aluminum pik on the scaffold.   NO ABATEMENT CERTIFICATION REQUIRED
