29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  a) Spring Area: On or about June 19, 2019, and at times prior, the employer exposed the technician to noise levels above the action level of 87 decibels (66% dose) without providing an audiometric test.  The employee had an exposure of 91.4 decibels (122.6% dose) during the 313 minute sampling period. The exposure calculation included a zero increment for the 167 minutes not sampled.

The employer did not institute a training program and ensure employee participation in the program:  a) Spring Area: On or about June 19, 2019, and at times prior, the employer exposed the technician to noise levels above the action level of 87 decibels (66% dose) without providing training. 29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:  Welding area: On or about June 10, 2019, and at times prior, the employer exposed employees to respiratory hazards in that a written respiratory protection program, to include training, a medical evaluation, and an annual fit test, was not developed or implemented for the welder wearing a 3M half face air purifying respirator.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Throughout the facility: On or about June 11, 2019, and at times prior, the employer exposed employees to chemical hazards, in that a written hazard communication program was not developed or implemented for employees working with chemicals, such as, but not limited to, Spirax 75W-90 oil,  Quick Color Spray Enamel, acetylene, and oxygen.
