East dba Mauser Packaging Solutions/South Plainfield, NJ - Throughout the facility: The employer did not provide audiometric testing to employees working in the drum and tote reconditioning facility and who were exposed to noise levels of 99.0 dBA (tote washer), 92.09 dBA (drum grinder operator), 91.47 dBA (tote line washer/shredder), 90.02 dBA (blowmold operator), 89.6 dBA (resins area foreman/eq. operator), 88.0 dBA (paint booth operator). These levels exceeded OSHA Action Level of 85 dBA for 8 hour TWA. Exposing employees to hearing loss. Condition noted on or about June 13, 2019.

Employee was exposed to 91.47 dBA for 8 hour TWA, which exceeded OSHA Action Level of 85 dBA for 8 hour TWA. Exposing employees to hearing loss. Sampling time was 386 minutes, and zero exposures were assumed for all unsampled time periods. Condition noted on or about June 13, 2019.

Employees working in the drum and tote reconditioning facility were exposed to noise levels of 99.0 dBA (tote washer) 92.09 dBA (drum grinder operator), 91.47 dBA (tote line washer/shredder), 90.02 dBA (blowmold operator), 89.6 dBA (resins area foreman/eq. operator), 88.0 dBA (paint booth operator). East dba Mauser Packaging Solutions/South Plainfield, NJ - Throughout the facility: Employees working in the reconditioning area including drum manufacturing and cleaning were using defective forklifts: a) The concentration levels of carbon monoxide (CO) gas created by the Nissan (orange/sit down type) forklift was 104 parts per million (ppm). b) The concentration levels of carbon monoxide (CO) gas created by the Nissan (white and orange/sit down type) forklift was 42 parts per million (ppm). c) The yellow, sit down, electric forklift had leaking battery acid. Condition noted on or about June 13, 2019.
