Drums were placed within the exit route limiting it's width to 18 inches, delaying employees exit in case of an emergency.    Condition noted on or about September 12, 2019.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    a) Recycle Inc. East dba Mauser Packaging Solutions/South Plainfield, NJ - Throughout the facility: The employer did not provide audiometric testing to employees working in the drum and tote reconditioning facility and who were exposed to noise levels of 99.0 dBA (tote washer), 92.09 dBA (drum grinder operator), 91.47 dBA (tote line washer/shredder), 90.02 dBA (blowmold operator), 89.6 dBA (resins area foreman/eq. operator), 88.0 dBA (paint booth operator). These levels exceeded OSHA Action Level of 85 dBA for 8 hour TWA. Exposing employees to hearing loss.     Condition noted on or about June 13, 2019.

29 CFR 1910.95(g)(5)(ii): Where mobile test vans were used to meet the audiometric testing obligation, the employer did not obtain a valid baseline audiogram within 1 year of an employee's first exposure at or above the action level:  a) Recycle Inc. East dba Mauser Packaging Solutions/South Plainfield, NJ - Throughout the facility: The employer did not provide baseline audiogram to employee working on the tote line section in the drum and tote reconditioning facility. Employee was exposed to 91.47 dBA for 8 hour TWA, which exceeded OSHA Action Level of 85 dBA for 8 hour TWA. Exposing employees to hearing loss. Sampling time was 386 minutes, and zero exposures were assumed for all unsampled time periods.    Condition noted on or about June 13, 2019.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:    a) Recycle Inc. East dba Mauser Packaging Solutions/South Plainfield, NJ - Throughout the facility: The employer did not provide annual training for employees included in the hearing conservation program and required to wear hearing protection. Employees working in the drum and tote reconditioning facility were exposed to noise levels of 99.0 dBA (tote washer) 92.09 dBA (drum grinder operator), 91.47 dBA (tote line washer/shredder), 90.02 dBA (blowmold operator), 89.6 dBA (resins area foreman/eq. operator), 88.0 dBA (paint booth operator). These levels exceeded OSHA Action Level of 85 dBA for 8 hour TWA, hence exposing employees to hearing loss.    Condition noted on or about June 13, 2019.

29 CFR 1910.178(p)(1): Powered industrial truck found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition:  Recycle Inc. East dba Mauser Packaging Solutions/South Plainfield, NJ - Throughout the facility: Employees working in the reconditioning area including drum manufacturing and cleaning were using defective forklifts: a) The concentration levels of carbon monoxide (CO) gas created by the Nissan (orange/sit down type) forklift was 104 parts per million (ppm). b) The concentration levels of carbon monoxide (CO) gas created by the Nissan (white and orange/sit down type) forklift was 42 parts per million (ppm). c) The yellow, sit down, electric forklift had leaking battery acid.  Condition noted on or about June 13, 2019.
