29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation.    On or about May 17, 2019 at 101 Hidden Trail Court, Holly Springs, Georgia , two employees installing siding on the supported scaffold (Pump Jack) were exposed to a fall hazard when the legs of the scaffold did not rest on base plates, mud sills, or other firm foundation.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.    On or about May 17, 2019 at 101 Hidden Trail Court, Holly Springs, Georgia,  two employees installing siding approximately 23 feet above the lower level were exposed to a fall hazard to the lower level.  Fall protection in the form of personal fall protection or guard rails and end rails were not provided.

29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform.    On or about May 17, 2019 at 101 Hidden Trail Court, Holly Springs, Georgia two employees installing siding from a supported scaffold (Pump Jack) were exposed to a fall hazard of approximately 23 feet to the lower level when there was no mid rail between the top edge of the guard rail system and the scaffold platform.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    On or about May 17, 2019 at 101 Hidden Trail Court, Holly Springs, Georgia an employees working below where two employees were  installing siding from a supported scaffold (Pump Jack) were exposed to a struck-by hazard  when not wearing a hard hat or addition protection was not in place to catch the debris, tools that fell.

29 CFR 1926.405(b)(3)(ii): Covers. Boxes shall be closed by covers securely fastened in place. Underground box covers that weigh over 100 pounds (43.6 kg) meet this requirement. Covers for boxes shall be permanently marked "HIGH VOLTAGE." The marking shall be on the outside of the box cover and shall be readily visible and legible.    On or about May 17, 2019 at 101 Hidden Trail Court, Holly Springs, Georgia an employee working at the temporary power pole was exposed to an electrical hazard when plugging in the compressor.  The cover on the panel box was not secured exposing the wiring within.

On or about May 17, 2019 at 101 Hidden trail Court, Holly Springs, Georgia, where employees were cutting Hardie Plank Concrete type siding for installation employees were exposed to the hazards associated with respirable crystalline silica.  The employer did not assess the hazard.

On or about May 17, 2019 at 101 Hidden trail Court, Holly Springs, Georgia, where employees were cutting Hardie Plank Concrete type siding for installation employees were exposed to the hazards associated with respirable crystalline silica.  The employees did not have a working knowledge and understanding of the requirements of respirable crystalline silica.
