On or about June 18, 2019, employees were exposed to fall hazards of approximately 4'-9" while working and operating forklifts on the dock because the south wall of the dock was not attached to the floor of the building. b. On or about November 15, 2019, employees were exposed to fall hazards of approximately 4'-2" because loading dock #5 located on the west wall of the dock was not guarded by a standard railing or its equivalent. The opening had a rope across the opening at a height of 32" that did not meet the guardrail requirements. c. On or about November 15, 2019, employees were exposed to fall hazards of approximately 4"-3" because loading dock #6 located on the west wall of the dock was not guarded by a standard railing or its equivalent. The opening had a rope across the opening at a height of 28" that did not meet the guardrail requirements. d. On or about November 15, 2019, employees were exposed to fall hazards of approximately 4'-3" because loading dock #13 located on the east wall of the dock was not guarded by a standard railing or its equivalent. The opening had a rope across the opening at a height of 18" that did not meet the guardrail requirements. e.
