29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.    The Lifeguard Ferroresonant Charger (Model:  18-1050F3BXV, S/N: B8904DJ) had the flexible metallic conduit pulled from charger with the electrical wires exposing employees to electrical shock hazards.
