Accent Metals, Inc. - On or about June 27, 2019, an employee working at the finishing belt machine was exposed to noise of 93.3 decibels on the A weighted scale (dBA) for an eight hour time weighted average while sanding the metal parts. b). Accent Metals, Inc. - On or about June 27, 2019, an employee working in the satin department was exposed to noise at 90.8dBA for an eight hour time weighted average while sanding the metal parts. c). Accent Metals, Inc. - On or about June 27, 2019, an employee working in the hand polishing department was exposed to noise at 88.8 dBA for an eight hour time weighted average while hand polishing and sanding the bumpers. d). Accent Metals, Inc. - On or about June 27, 2019, an employee working at the Harper/aluminum extrusion machine was exposed to noise at 88.7dBA for an eight hour time weighted average while sanding the metal parts. e). Accent Metals, Inc. - On or about June 27, 2019, an employee working at the Harper/polishing machine was exposed to noise at 87.8dBA for an eight hour time weighted average while sanding the metal parts.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a). Accent Metals, Inc. - On or about June 27, 2019, an employee working at the finishing belt machine was exposed to noise of 93.3 decibels on the A weighted scale (dBA) for an eight hour time weighted average while sanding the metal parts. b). Accent Metals, Inc. - On or about June 27, 2019, an employee working in the satin department was exposed to noise at 90.8dBA for an eight hour time weighted average while sanding the metal parts. c). Accent Metals, Inc. - On or about June 27, 2019, an employee working in the hand polishing department was exposed to noise at 88.8 dBA for an eight hour time weighted average while hand polishing and sanding the bumpers. d). Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.1026(l)(1)(iii): The employer had not included chromium (VI) in the hazard communication program, established to comply with the Hazard Communication Standard, 29 CFR 1910.1200, ensured that each employee had access to labels on containers of chromium (VI) and to safety data sheets, and was trained in accordance with the requirements of the Hazard Communication Standard and 29 CFR 1910.1026(l)(2), including the contents of the Hexavalent Chromium Standard, the purpose and a description of the medical surveillance program, and made copies of this standard available to all affected employees: a). Accent Metals - The employer did not include information about hexavalent chromium in the hazard communication program nor did the employer provide information and training on the hazards of hexavalent chromium to an employee who tig weld small tabs on sheets of stainless steel, on or about June 19, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
