Glenwood Avenue, Philadelphia, PA 19133 - A written hazard communication program was not developed or implemented in the workplace where employees used hazardous chemicals, thus exposing employees to health and physical hazards of hazardous chemicals, including, but not limited to:  - Calcium sulfoaluminate cement, and silica sand, quartz, contained in Rapid Set Concrete Mix; - Lead, contained in lead ingots; - Liquified petroleum gas (propane), contained in propane cylinder.  This condition was observed on June 18, 2019.
