29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about June 20, 2019,  three employees were not protected by eye or face protection while using impact drills to install pieces of sheet metal roofing, exposing the employees to the hazard of being struck by flying debris.

29 CFR 1926.760(a)(1): Each employee engaged in steel erection activity who is on a walking/ working surface with an unprotected side or edge more than 15 feet above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.  On or about June 20, 2019, one employee installing steel metal roofing on the steel building under construction was not connected to the anchor ropes provided, exposing the employee to a fall hazard of approximately 24 feet.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  On or about June 20, 2019, and at times prior thereto, a metal platform/basket installed on the JCB 508 Loadall  rough terrain forklift was used to elevate personnel to perform work on the commercial property under construction. The forklift was not designed for such use per the manufacturer's operations manual, exposing employees to the hazard of falling approximately 20 feet.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  On or about June 20, 2019, one employee was working from an elevated metal platform/basket that was not securely attached to the lift to prevent the platform from sliding off. The platform was used with the JCB 508 Loadall rough terrain forklift to access elevated positions on the commercial property under construction, exposing employees to a fall hazard of approximately 20 feet.
