The overhead restraints were not in use by employees and no other form of guarding was in place. The brake was operated by foot pedal.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows, floors, or similar openings;    On or about June 19, 2019 the employer failed to ensure the electrical flexible cords were not run through wall holes at the facility. A Flexible cord providing 110v power to the metal cut off saw was ran through a hole in the wall to the electrical outlet on the other side.
