29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  On or about May 25, 2023 the employer did not ensure that walking and working surfaces between the Lucas and Fermat horizontal boring machines were free of trip hazards. The walking area contained metal control cable covers which protruded up from the floor presenting the hazard to employees.

29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  On or about May 25, 2023 the employer did not ensure that employees were prevented from stepping or tripping into the metal chip and chemical trough of the Fermat horizontal boring machine. The trough did not contain a cover between the Fermat and the Lucas horizontal boring machine to protect employees from the hazard.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about May 25, 2023 the employer did not ensure that employees were protected from flying chips while operating the Lucas horizontal boring machine. The employer did not ensure that a removable  chuck/ chip guard was in place during operation.   b) On or about May 25, 2023 the employer did not ensure that employees were protected from the pinch point created by the Fermat horizontal boring machine and structural components of the facility. The automatic tooling station of the machine automatically operates, travelling north and south. Employees are exposed to a pinch point between the tooling station and a building column when the station travels south toward it's "home" area.
