The employer did not ensure that a removable  chuck/ chip guard was in place during operation.   b) On or about May 25, 2023 the employer did not ensure that employees were protected from the pinch point created by the Fermat horizontal boring machine and structural components of the facility. The automatic tooling station of the machine automatically operates, travelling north and south. Employees are exposed to a pinch point between the tooling station and a building column when the station travels south toward it's "home" area.
