29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.                                                                                                             a) Platforms 1 - 6 - On or about July 12, 2019, employees placing solution inside a cooker on the  work platform were exposed to fall hazards in excess of six feet when the gate was left open and as the employees closed the gate.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically outline specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    On or about June 12, 2019, employees perform maintenance and service on various equipment.  Employees were exposed to struck by, caught in and amputation hazards in that:      a) Extrusion line 1-8, Thermal Energy 200F- The procedures do not indicate how much time employees need to wait until the machine cools off, it also does not indicated the PPE to be used.       b) Extrusion line 1-8, Kinetic Energy 1800 RPM- The Procedures does not indicate how long to wait for equipment to come to stop and addition it does not indicate the location and how to use the block or chain to stop equipment from moving.
