29 CFR 1910.265(c)(26)(ii): Lumber lifting devices. Lumber lifting devices on all stackers shall be designed and arranged so as to minimize the possibility of lumber falling from such devices. a) On 6/19/2019 an operator utilizing a lumber lifting device placed a 6-7 ft. log onto a conveyer system, the log fell into an opening in the conveyer system framing resulting in the log becoming lodged. An employee was in the process of trying to dislodge the log, while trying to free the trapped log, an employee asked the operator to use the log lifter to try to reposition the log, as the lifter touched the log, it caused the log to shift catching an employee's right hand in between the log and the frame of the conveyer system.

29 CFR 1910.265(c)(26)(ix): Inspection. Every stacker and unstacker shall be inspected at frequent intervals and all defective parts shall be immediately repaired or replaced. a) On or about 6/19/2019 the employer did not conduct the frequent inspections required on each stacker to discover defective issues. On 6/19/2019 a 6-7 ft. log had falling through an opening in a conveyer system, the employee tried to reposition the log, asked the operator to assist by using the stacker to reposition the log, as the stacker came in contact with the log, the log shifted and caught the employee's right hand between the conveyer system and the log.
