On or about 17 Sep 2019, the chain guardrail was not installed exposing workers to a fall hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) Lumber Yard "Temple", located at No 1, KY RT 2541, Greenup, KY.  On or about 17 Sep 2019, the barrier guard on the conveyor belt motor was damaged creating large openings on three sides exposing workers to rotating parts and pinch-point hazards.

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy.   a) Lumber Yard "Temple" - walkway, located at No 1, KY RT 2541, Greenup, KY.  On or about 17 Sep 2019, The electrical outlet providing power to the pump heater did not have a cover exposing energized parts.   b) Lumber Yard "Temple" - walkway, located at No 1, KY RT 2541, Greenup, KY. On or about 17 Sep 2019, The electrical switch used to turn the heater for the pump did not have a cover exposing energized live parts.  c) Lumber Yard "Temple" - walkway, located at No 1, KY RT 2541, Greenup, KY.  On or about 17 Sep 2019, The relay switches for the pump was missing the cover exposing energized live parts.
