29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems:      a) Carr St., Littleton, CO: On or about June 21, 2019, the competent person at the site did not identify significant deficiencies at the excavation site, such as but not limited to lack of cave-in protection, improperly placed spoil piles, and unsafe ladder access prior to allowing workers to enter the excavation.
