29 CFR  1926.651(k)(1): An inspection was not conducted by the competent person to identify evidence of other hazardous conditions as needed throughout the shift:  a)  Bison Ridge Construction LLC., at E. 110th Ave & Turnberry Pkwy., Commerce City, CO 80222: On or about September 1, 2021, and at times prior, the competent person at the site did not identify significant deficiencies at the excavation site including missing cave -in protection at the end of a vertical cut trench where workers were working and in which ground water was accumulating,  tabulated data for the shoring in use was not maintained at the jobsite, and the tabulated data that was eventually found did not match the shoring in use. This exposed employees to a crushing hazard by excavation wall cave-in.

A worker sustained a partial finger amputation when he was setting up a ladder and pinched his finger.   Employer failed to protect employees from finger amputation and crushing hazards.
