Prairie Village, Kansas:  An employees engaged in ground hand activities working below employees engaged in roofing activities was exposed to struck-by hazards in that head protection was not provided.

Prairie Village, Kansas:  Employees engaged in residential roofing activities using a circular saw were exposed to struck-by hazards in that eye protection was not provided.

Prairie Village, Kansas: Employees engaged in residential roofing activities from a 4:12 pitch roof were exposed to fall hazards of approximately 8 feet 6 inches from the eave of the residence to the ground below in that fall protection was not provided.
