29 CFR 1926.416(a)(2): In work areas where the exact location of underground electric power lines was unknown, employees using jackhammers, bars, or other hand tools which may contact a line were not provided with insulated protective gloves: Near the gymnasium at FCI El Reno, Oklahoma. On or about May 17, 2019, an employee was attempting to move a new underground water line with a approximately 6 foot metal pry bar when he contacted a 12.5 kV underground power line. The employee was not using electrical insulated gloves and exposed to the hazard of electrical shock.

29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: Near the gymnasium at FCI El Reno, Oklahoma City. On or about April 29, 2019, and at times prior thereto and thereafter, the employer did not ensure that employees were provided with safe and acceptable means such as electrical insulating gloves and non-conductive tools when determining the exact location of unground utilities, exposing the employees to the hazard of electrical shock.
