On or about June 27, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about June 27, 2019, at the above addressed jobsite, employees performing residential roofing operations, twenty (20) feet above the ground, were not protected from falls, thereby exposing employees to fall hazards.
