29 CFR 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames being used for access had a spacing greater than 16 3/4 inches: a- 1302 - 5th Avenue, Asbury Park NJ: On or about 06/27/19, a company employee was exposed to fall hazards in excess of 10 feet to the ground surface below when accessing and egressing the working levels of supported scaffolds using end frame openings that exceeded 16 3/4 inches. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a- 1302 - 5th Avenue, Asbury Park NJ: On or about 06/27/19, a company employee was exposed to fall hazards in excess of 20 feet to the ground surface below when working and walking from the top buck of a supported scaffold that was not provided with the required guard rail systems and this employee was not using a personal fall arrest system as an alternative. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
